NFTI: One Key to Self-Determination in LTC
Michigan is creating a long term care system that is more responsive to customer choice and need for community-based supports. One critical part of increasing choice is allowing persons in nursing facilities to decide whether they wish to remain where they are or move to a home of their own in their community of choice. NFTI is about choosing to live in the community.
Transitioning from an institution to the community of choice can be a daunting task. The NFTI Project has created a training curriculum to help persons in transition, their families, and their support networks effectively and safely enable relocation to the community of their choice. This curriculum is presented here, in its entirety, as a series of training modules.
The Training contains an enormous amount of material, including supplements in the Appendix. Accessible versions of all materials are located in each of the modules. So, take your time, explore, and come back to check out what you didn't have time for before. Transition and Diversion are a core of rebalancing Michigan's Long Term Care system, and building choice in LTC for all of Michigan's citizens who need it.
